Reporting, Investigation, and Analysis

2-Day Course

This is a two-day course made up of four components—Hazard Reporting, Incident Reporting, Hazard and Incident Investigation, and Hazard and Incident Report Analysis—each of which is completed in half a day.

Hazard Reporting and Incident Reporting (day one)

A hazard is a workplace feature or obstruction that may cause injury to a workplace employee or visitor, and an incident occurs when an injury is caused or when an injury is only just avoided in a "near miss." Participants enrolled in this course will learn how to:

• meet their responsibilities in the event of a hazard or incident under current OHS legislation;
• identify hazards and deal with incidents when they occur;
• develop a hazard and incident report template; and
• write hazard and incident reports in accordance with current OHS legislation and the OHS policies of their employers.

Hazard and Incident Investigation and Report Analysis (day two)

Wherever a hazard is identified or an incident occurs, the law requires that an investigation be conducted and that its findings be reported. Investigation reports can then be analysed in order to identify safety trends in a particular workplace, which then allows workplace managers to implement preventive risk control measures and to proactively ensure the safety of the work environment. Participants enrolled in this course will learn how to:

• conduct investigations in accordance with legislative requirements;
• complile reports based on the findings of those investigations;
• analyse a number of reports in order to implement effective risk control measures; and
• use data acquired through analysis to identify further training requirements for workplace staff.
